Have you heard the news? OSHA has issued a final rule on Walking-Working Surfaces and Personal Fall Protection Systems (29 CFR 1910 Subpart D&I) that goes into effect January 17, 2017. 3M Fall Protection (formerly Capital Safety) has identified a limited number of 3M™ DBI-SALA® LAD-SAF™ X2 and 3M™ DBI-SALA® LAD-SAF™ X3 sleeves with the locking lever not working properly. This condition can allow the sleeve to disengage from the cable during use. The assembly error is believed to affect only a limited number of units and has been corrected. There have been no accidents or injuries related to this issue.

Please refer to the Inspection Notice (English) (Spanish). The Notice provides instructions to end-users on how to inspect their units for this condition and how to return affected sleeves for replacement.

Tower Techs’ Lives Are Miraculously Spared – Literally

When crew members of Dynamic South, Inc. were working on a 350-foot self-supporting SBA Communications tower site providing upgrade services for AT&T at about the 190-foot level, the Allegiance crane, according to police, collapsed, narrowly missing workers. It is not known whether techs were on the structure at the time when the crane overturned.

Tower Tech Dies After Falling From a 160-foot Oklahoma Monopole

Steven Lamar Hill, a 44-year old tower technician from Chandler, OK, died after falling from a 160-foot tall monopole on Nov 2, 2016. It is not yet clear what caused Hill to fall. The employer, Mann's Tower Services, while having been cited for serious violations, has never been cited for improper training or improper use of fall protection equipment.
