This information helps provide community estimates
for COVID-19 vaccine age tiering

  • LVHN needs to expedite a survey to identify counts/volume for all patient age groups to prepare for the full community rollout that will take place over the coming weeks and months
     
  • These estimations allow LVHN to request vaccines from the state based on our needs.
     
  • 75 and older will be the initial priority age group to be vaccinated in the community.

Vaccine Rollout Plans to those 75 and older


Community members that are 75 and older will soon be eligible to get vaccinated. Details regarding the Community Roll Out will be shared as soon as available and approved.

Here are helpful steps to advise your patients:
  • Sign up for a MyLVHN account or
  • If the patient does not have the capacity or tools to create a MyLVHN Account directly, encourage them to ask a designee to create it for them.
  • Call 888-402-LVHN (5846) for help setting up an account.
  • Vaccines will be scheduled through the MyLVHN portal.

What to tell patients who want to get the vaccine:
Create a MyLVHN Account

The easiest and fastest way to schedule a vaccination is through LVHN’s patient portal, MyLVHN. Those who do not have a MyLVHN account can create one at MyLVHN.org. You do not need to be a current patient of LVHN, LVPG or Coordinated Health to open a MyLVHN account.
