At LVHN, we value all of our care providers. Your practice has been identified by LVHN to receive the COVID vaccination and all of your staff may now schedule. Please disregard if you and your staff have already been vaccinated or have a scheduled appointment.
MyLVHN portal is the best way to schedule vaccines
To schedule vaccinations, please have your staff check their MyLVHN portal (if they already have one) for an option to self-schedule. Select the desired location and choose a 15-minute appointment slot. The process is simple.
If you do not currently have a MyLVHN account please click the following link to sign-up now https://www.mylvhn.org/MyChart/signup. This portal is the easiest way to schedule your vaccination, communicate, and receive other information.
Call to schedule
Initial vaccine appointments can also be scheduled by calling 484-884-1767. The scheduling office has a list of Identified practices. To create an appointment staff simply supply their full name, practice name, and date of birth.
