The ‘Personalised Care and Support Planning Handbook’ is designed to spark new thinking amongst health and care practitioners about the best ways to locally implement personalised care and support planning.

The handbook provides practical support for local health systems hoping to introduce and sustain care and support planning at a local level and give people with long term conditions more control over their lives.

Extending Our Reach

We have started 2015 as we mean to go on, signing up nearly 60 new members to the Coalition at the Long Term Conditions Conference in London.

Our Deputy Director, Catherine Wilton (pictured right), ran a very successful workshop accompanied by members of our Co-Production Group.

The session focused on what would need to happen to make person-centred collaborative care a reality for people with long term conditions.

Catherine said: " The conference was really encouraging. The workshop, and all of our conversations outside of it, indicated that there is real appetite for changing how the health service as a whole responds to people living with long term conditions."
