LGH E-Notification of Admission and Death: FAQs

It has been two months since the e-notification of admissions and deaths went live. We hope that you are finding these reports helpful. We acknowledge that faxing back with pertinent patient information adds to your already busy day of work, so thank you for your help! We are confident that, ultimately, the patient, hospital physicians, and you, the community GPs, will get much more back in return. Please know that we are exploring ways to have your EMR make this task easier for you.
Have questions? Please have a look at the FAQ above. We have also attached the Medical Orders for Scope of Treatment (MOST) form here. For further information, please contact Chisato Ito, Project Lead, at cito@divisionsbc.ca.

Sincerely,
GP/Hospitalist Working Group

Call Groups

The Division would like to help connect physicians who are seeking a call group, and call groups seeking new members. For further information, please email northshore@divisionsbc.ca.

Pathways Update

Pathways now includes data for more than 3,500 specialists, 900 clinics and hospitals, 1,200 resources, and 1,500 referral forms across the province! New resources and features are frequently added to Pathways. If you are not already using Pathways in your practice, take advantage of this very useful database by contacting us at northshore@pathwaysbc.ca.

Are You a Family Physician with Special Interest?
Pathways Family Practice specialty is designed to provide a list of GPs who take referrals for specific services. For example, some GPs who do not do primary care obstetrics, or skin biopsy, will rely on their GP colleagues to provide these services when they have a patient who needs them. If you provide specialized services to the patients of your GP colleagues, and you wish to have a searchable profile on Pathways, please contact us at northshore@divisionsbc.ca

Seeing Inaccurate Wait Times?
Pathways is built on the idea of communal ownership, and a recognition that Pathways is only as good as the accuracy of its information. If you see information about a medical provider or clinic that you feel is incorrect or missing, we strongly encourage you to simply press the “Incorrect Information? Let us Know” button or “Contact us” tab at the bottom of each page and enter comments. We will review your comments, and act on them as appropriate.

JCC Event on March 1, 2017: "Simplifying the Journey”

The Joint Collaborative Committees (JCCs) have partnered with the BC Patient Safety and Quality Council's Quality Forum 2017 to offer “Simplifying the Journey”, a pre-forum day of interactive workshops on March 1, 2017 (approved for up to 6.0 MOC Section 1 and 6.0 Mainpro+ study credits). This event will enable family physicians, specialists, allied health providers, policy leaders, patients, and families to learn from each other and align efforts in building a truly integrated system of care. The day focuses on simplifying the health care journey around three key health system priorities, recognizing that all elements connect and intersect with each other:
•    Primary Care Journey
•    Surgical Care Journey
•    Rural Care Journey
The JCCs will cover the cost of registration and provide sessional payments for practicing physicians to attend Simplifying the Journey. Please contact JCC@doctorsofbc.ca for a complimentary registration code, and then click here to register.
