Welcome to our special resource edition of ACCESS, a monthly digest of transition-related news, perspectives from diverse voices, upcoming events, and resources for young people with disabilities in New York State.

Below you will find a series of clear, thoughtful, easy-on-the-eyes tip sheets composed by our experts on various transition subjects— including high school graduation, disability disclosure, and housing — that affect our community.

You'll find links to the same tip sheets in Spanish below as well. Please pass along to those who might need them.

These tip sheets are brought to you by NYS Transition Partners: a collaborative project funded by the Rehabilitative Services Administration to help NYS families and young adults with disabilities navigate the transition to productive, independent, and meaningful adult lives.

Mental Health and Transition Age Youth

Young people with disabilities face many challenges when they age out of the public education and youth service systems. There are fewer resources, less available support, and proper diagnosis and treatment become even more difficult for adults.

This webinar will explore the topic of mental health for transition-aged youth, share related resources, and provide next steps.
